Respirator Medical Evaluation
The OSHA 1910.134(e) medical evaluation, online. $50 per person.
OSHA requires a medical evaluation before anyone is fit tested or wears a respirator on the job (29 CFR 1910.134(e)). Each person fills out an online questionnaire, about 15 minutes, and a physician reviews it. A clinic visit is usually not needed. There is nothing to schedule.
What Happens Next
- Pay by card.
- Fill out the short hazard form linked in your receipt.
- We email the questionnaire link, and each person completes it online.
- The physician's result is emailed to you.
Common Questions
Do loose-fitting PAPR hoods and helmets need a fit test?
No. A loose-fitting hood or helmet has no face seal to test, so OSHA does not require a fit test (1910.134(f)). Anyone who wears one still needs this medical evaluation plus the annual respirator training, not a fit test.
Does this include a fit test?
No. This is the medical evaluation only. Tight-fitting respirators, such as N95s, half masks, full facepieces and SCBA, also need a fit test. Book a fit test and add the medical evaluation there.
What if someone is not cleared?
The physician may ask for more information or a clinic visit before clearing a person. If that happens we will tell you what is needed. The evaluation fee is not refunded once the physician has reviewed it, and a local occupational health clinic may still be able to clear them.
Do we need to schedule anything?
No. After you pay, fill out the short hazard form linked in your receipt. We then email the questionnaire link and each person completes it online, about 15 minutes. A clinic visit is usually not needed.
Can I get a refund?
Refunds: if you cancel, we refund each person's evaluation until a physician has reviewed it. Once it is reviewed, that fee is not refunded.
